1.  Recognize and diagnose an incarcerated and strangulated umbilical hernia based on clinical presentation and diagnostic tests.   

2.  Discuss the importance of the distinction between an incarcerated hernia and a strangulated hernia.

3.  List the risk factors and complications associated with a strangulated umbilical hernia.

4.  Discuss the treatment needed for a strangulated umbilical hernia.   

5.  Define sepsis according to the 3rd International Consensus Definitions for Sepsis and  Septic Shock (Sepsis-3), and explain why the use of 2 or more SIRS criteria to define sepsis has been abandoned.

Boerhaave Syndrome

By the end of this scenario the learner will be able to:

1.  Define Boerhaave Syndrome.

2.  Describe the criteria for sepsis and septic shock.

3.  Discuss appropriate antibiotics and fluid resuscitation for a critical patient with sepsis.

4.  Describe anchoring bias with regards to patients with substance abuse.

5.  Discuss the appropriate treatment and surgical/specialty consultations needed for a patient with Boerhaave Syndrome.

Acute cholecystitis

By the end of this scenario the learner will be able to:

1.  Recognize and diagnose acute cholecystitis according to the World Society of Emergency Surgery (WSES) Guidelines.

2.  Describe the abdominal ultrasound findings that can be seen in a patient with acute calculous cholecystitis. 

3.  Discuss appropriate antibiotic coverage, supportive care and definitive treatment for a patient with acute calculous cholecystitis. 

4.  Develop a list of possible complications of acute cholecystitis that could occur in the clinical setting.

5.  Describe the necessary consultants and disposition for a patient presenting with acute cholecystitis.
